In response, an expert UK consensus group aimed enhance current guidance. Objectives To develop evidence- consensus-based set recommendations for routine practice that adds value suggests minimum best standards day-to-day provides practical strategies achieve these standards, aim supporting clinicians. Methods A steering committee (SC) was formed 12 experts fields rheumatology, dermatology primary care. At initial meeting, themes were discussed agreed (PsA diagnosis, disease assessment, comorbidities, management) questions drafted. targeted literature review PubMed Embase following PICO framework conducted gather scientific evidence selection questions, while others be more appropriate answer using experience. second drafted response covering guidelines. For next stage, extended faculty recruited including rheumatologists (32), dermatologists (6), representatives (3), specialist nurses (5), academics (12) members Brit-PACT patient [6]. online voting platform, SC entered agreement score each recommendation. Consensus achieved when 75% respondents 7–9 on 9-point scale. Results The 34 statements 15 management. theme focussed identify early refer appropriately, assessment diagnostic indicators, use screening tools. imaging support referral time frames also covered detail. Recommendations centred holistic consideration activity, impairment impact from perspective, well how implement shared decision-making. Specific measurements performed at clinic visits included. made specific high-impact conditions, depression obesity, implications therapy. Management (which excluded therapies extant guidelines) multidisciplinary team working, implementation lifestyle modifications, treat-to-target built upon parameters greatest importance individual patients. corticosteroids recommended minimised where feasible. Evidence-based Recommendations for the Management of Comorbidities in Rheumatoid Arthritis, Psoriasis, and Psoriatic Arthritis: Expert Opinion of the Canadian Dermatology-Rheumatology Comorbidity Initiative.
OBJECTIVE Comorbidities such as cardiovascular diseases (CVD), cancer, osteoporosis, and depression are often underrecognized in patients with rheumatoid arthritis (RA), psoriatic arthritis (PsA), or psoriasis (PsO). متن کاملCardiometabolic Comorbidities in Psoriasis and Psoriatic Arthritis
There is solid epidemiologic evidence linking psoriasis and psoriatic arthritis (PsA) to cardiovascular risk factors and an increased risk of developing cardiovascular disease. Chronic inflammation, with shared pathways and cytokines common to metabolic syndrome, atherosclerosis and psoriasis, might provide the basis for the cardiovascular and metabolic comorbidities of psoriasis and PsA. The p...
